This is a friendly reminder to ensure you are scheduling your follow-up appointments as needed. Regular visits allow Dr. Lyday to continue providing the highest level of care and help you stay on track with your health goals.
Ongoing Care Requirements to Remain an Active Patient: If more than 12 months have passed since your last visit, a 60-minute re-establishment appointment is required. This allows Dr. Lyday adequate time to review updates to your health history and current concerns.
Patients who are prescribed pharmaceutical medications must be seen at least twice per year for a minimum of 30 minutes to continue receiving medication refills.
Dr. Lyday is currently booking approximately two months out, so if you anticipate running low on refills, please plan ahead and schedule early. This helps ensure a smooth process and avoids interruptions in your care.
